Putting behaviour support plans into practice

A behaviour support plan only works if it's followed day to day. We're an implementing provider: we don't write plans. We work alongside the participant's behaviour support practitioner so the strategies in the plan happen consistently, at home and out in the community.

How we work with the plan

  • Workers read and follow the behaviour support plan, and we take time to get to know each person, their routines and the people around them.
  • Consistent, familiar workers, not someone new every few weeks, and steady routines so each day feels predictable.
  • Workers record the data the practitioner asks for, and write incident notes when something happens.
  • We flag changes early, so the practitioner can review the plan before small issues grow.
  • Regulated restrictive practices are only used as authorised in the plan.
  • Regular updates for the participant, family, practitioner and support coordinator.

  2. We call you back the same day

    We'll ask about goals, needs, the plan and who the practitioner is.

  3. Fartun meets you

    Fartun meets the participant and family to talk through routines, preferences and anything that matters to you.

  4. We match consistent workers

    We look at what the person wants, such as gender, culture, language and personality, and aim for the same familiar faces each week. We keep you, your practitioner and your support coordinator updated as things progress.

Frequently asked questions

Do you write behaviour support plans?

No. The plan is written by a behaviour support practitioner. We put it into practice day to day and feed back what we see.

What if there's no plan in place yet?

Talk to us anyway. Your support coordinator can help you find a behaviour support practitioner, and we'll work alongside them once a plan is in place.

How will the practitioner know how things are going?

Workers record the data the plan asks for and write incident notes, and we share regular updates and flag changes early.

Can I use this if my plan is self-managed or plan-managed?

Yes. We work with NDIA-managed, plan-managed and self-managed participants.
